To order books by mail and pay by check or money order, please print
this page, fill it out and mail it with payment (check or money order
only, payable to Shelly Thacker) to
the address at the bottom. Shipping and handling is just $4 per order to
any U.S. address, no matter how many books you buy.* Quantities
are limited. Sorry, but we are unable to accept
credit cards for mail orders.
You can also order tapes at the same
time, place both forms in a single envelope, and pay with one check. Note:
pay only one $4 shipping fee for your combined order. Contact
us if you have any questions.
If you would like autographed copies, please PRINT the first name of
the person to whom each book should be signed. If you do not want the
books autographed, please write NO AUTOGRAPH in the "Autograph
To" column. Contact us if you have any

You can obtain one from any U.S. Post Office for just 90 cents. If you pay
by U.S. Postal Money Order, your books will be shipped the day we
receive your order. If you pay by check, your books will be shipped after
your check clears.
International customers please note: All prices
are in U.S. dollars.
